Request to Amend Education Records

 

As permitted under the the Family Educational Rights and Privacy Act (FERPA), current or former Johns Hopkins University (JHU) students may use this form to request amendment of JHU education records that they believe to be inaccurate, misleading, or otherwise in violation of their privacy rights.

JHU will respond to requests to amend education records in a timely manner.

Are you a current or former student with an active JHED ID? 

Please note, students who do not authenticate using an active JHED ID when submitting this form will be contacted to schedule a virtual appointment during which they will be asked to display a valid photo ID as proof of identity. JHU cannot move forward with requests until the student identity is verified. If a student does not respond to a request for identity verification within 30 calendar days, their request will be closed.
